Aminoglycoside

Aminoglycoside: conc dependant cidal
Gentamicin,Neomycin, Streptomycin,
tobramycin, Amikacin
-bind to 30S prior ribosome formation & distort its structure (interfere with initiation pr synthesis) , & /or misreading of mRNA causing mutation or premature termination
-enterococcus (with ? lactam), aerobe G- bacilli( Pa, Kl pn)

-Highly polar SO given parenterally
(Neomycin topical )

SE:
*Dose related toxicity
*Nephrotoxicity & ototoxicity (deafness)
*Allergic reaction ( neomycin )

Uses:
1-P a (tobramcin + anti pseudomonal P)
2-Klebsiella (gentamicin + anti pseudomonal P)
3-Brucilla (gentamicin + doxycycline)
4-Enterococcus ( Gentamicin +Pen G)
Macrolides
Erythromycin, Clarithromycin, Azithromycin,
Telithromycin
-Bind irreversibly to 50S & inhibit transpeptidation

-are enzyme inhibitor (except Azithromycin)
Erythromycin:
-The same organism as Pen G
